Requesting Information

You may request information from the Supervisor of Elections office by filling out the forms below and delivering them to us.

To purchase a list:

You must pay the cost of processing and delivering (postage) the data.

Candidates must pay with a campaign check, Committees with a committee check, Governmental agencies with an official check.

You must complete the appropriate order form.

There is a minimum order fee of $5.00

Electronic File (CD-ROM)	$10.00
Xerox Copies	$.15 per Page
Demographic Information	Free

Any Absentee data request costs $20.00 per election. Please contact our office for further explanation and ordering Absentees.

Allow 5 business days for processing. All prices are subject to change.

Orders are taken on a first come, first serve basis. Your order is assigned a number when properly received. You will be called when the order is ready. Orders received after 3:00pm will be considered as next day receipt. All orders must be placed on the appropriate order form. NO PHONE ORDERS!!! Faxed orders will be accepted as long as the original order form is delivered to us also.

All printed label orders are limited to a maximum of 25 pages (500 labels).
